On the Tableau Server main page, there is a question mark icon in the top-right corner. When you click it, three menu options appear. Is it possible to customize this menu? Specifically, I would like to add a link to our internal user manual as an additional item in this dropdown.

Probably the one most in line with what you describe is putting help text and a link on the home page in the welcome section: "Add a custom message to the welcome banner on the home page for all server users to see. The custom message can contain up to 240 characters of text and hyperlinks as well as one paragraph break. Administrators can also disable the default Tableau welcome banner for the server. To set a custom welcome banner, sign in to a site on Tableau Server. On the left-side navigation pane, select Manage all sites from the drop-down site list. Select Settings, then navigate to the Customization page.". We made use of this for some notes to help users after a version upgrade:
The other option I see talked about a lot is to have a standard dashboard format where there is a help link in a specific place on each dashboard that people get used to being able to use.
I haven't heard of repurposing or adding to the top right help icon drop down menu I'm afraid.
