The "ContactName" screen component isn't properly configured. i am getting this error while trying to launch in Collect Contact Info from Your User Module ,Please help

I also have a problem with this. Did you figure it out? for me it says

The "ContactName" screen component isn't properly configured. Check the instructions.

When I followed the instructions accordingly!
