I so love this collaborative concept! In the United States, the Colorado Office of e-Health Innovation has open a Regional Social Health Information Exchange (SHIE) Hubs Request for Application that is looking for proof-of-concept projects that includes "Connecting individuals to safe, affordable housing.". Perhaps there are firms in this group who might be interested in that opportunity. Please share with Salesforce Housing Sector as I'm not able to. @Nonprofit and Education MindShare
