360 Degree view of a complex patient

If a patient has multiple diseases and conditions with their attendant problems, goals, tasks (and possibly CareTeams???) - 

Is there a best practice for a single Patient Care Plan that is built by combining hte details from multiple CP templates?  (requireig a customizatino to add child records to the single CP from an additional CPT)?

Or multiple Disease-Specific Care plans, with customization to create a single view that encompasses all?
