How are other Health Cloud users getting around the issue where the Patient Accounts are shared with Leadership and users above them in the Role Hierarchy?

We created an operational hierarchy within the patient accounts to solve for reporting.
In all of our owned entities it was determined that a clinical employee should have access to a full record and it was their obligation only to access records based on need under hipaa.
In our MSA arrangements we had to create a separate patient/person account because that need to know was isolated to a geographic region. Also if that relation were to end, we would not own that patient data and separating it would be near impossible.
