I am struggling trying to find the formula to add the fields to the patient care to complete Challenge 6. Any suggestions?

Hi All,
I think I am getting really close on this. I'm just missing the condition field. Listed below is a screenshot of the fields I am using. Can somebody take a quick look and let me know what is required to allow a save. More importantly for a pass.
