Logins not recorded in login history or audit trail??

Is it at all possible for a user's logins to not be recorded in (or to get erased from) their login history and audit trail, or are those really fool-proof? We have a user who says she was working and saving her work during a 10 day period in which she has no login history, and the saved work is also not saved. Support said that the audit trail is the ultimate record, but when I looked at the 6-month audit record for our company, it did not display all of anyone's activity.

    Are you still seeing this issue? I ask because I have a user stating the exact same time. I have looked at her login history and see she logged in 4/25/16. Surprise she created a new account on 5/6 and 4/29 which I see thru reporting.
