All of our patient go through the same onboarding phase during their first 2 weeks on enrollment.

I have created a Care Plan Template with two related Care Plan Template Tasks.  I would like this to be the default care plan that gets assigned to all new patients when converted from a Lead object.  I know the care plan gets created in a the Process Builder job I have, but I can't find any information on how to customize the care plan assignment.

Because the "Create Care Plan and Care Team" Apex class is a managed entity, I can't view the code to see what fields are available in the class.  

Any pointers?
