How do i display my data in this specific format mentioned in the attachment?

Attached is packaged workbook created in Tableau 10.1.5. Please review the 'Sample data' and 'Required format' attachments and let me know how i can display my data in the specified format. I am open for any way possible.

In my data i have coupon numbers and the dates when those coupons were issued to customers and redeemed by customers. I would like a view/pivot of this data to show count of coupons issued and redeemed by their corresponding dates. Thanks!
