Can someone tell me if an outbound change set from Sanbox to prod will update values in a picklist if the field already exists or will it try and create a new field?

Think of it as being Overwritten ...
If you deploy the field, the field properties will be overwritten with the values you're deploying with.
if the field does not exist in the production , it will be created.
