Requirement #1:
I want to grant some Roles READ ONLY access to all N&As.
I want to grant some Roles READ/WRITE to only their N&As.
What I've found:
The only security is using the Private checkbox, which only applies to the owner & administrators, or those with View All Data on their profile.
There are no Sharing rules.
I could create a custom object, but that would only solve the Notes issue, and not address the Attachments.
Does anyone know of creative work-arounds?

Hi Jackie,
Notes and Attachments are a little funky in that their aren't permissions associated with them directly. Instead the permissions are determined based on the parent object which can cause some issues.
I suggest instead of Notes & Attachment you use Chatter Files and upload your attachments to chatter files.
Loading to Chatter files will let you take control on who can and cannot modify the attached file, it will also provide information on updates to the files if any on a real time basis to all the users following that record.
