Dynamically loaded smart form in landing page.

Hi Team,  

Can anyone please help to the below requirement?  

Requirement:- 

There are two different smart forms in one landpage. So, need to display dynamically smart form  based on attribute value.

 eg:- 

1) if attribute value is open, that the Smartform-1 should be loaded   

2) if attribute value is close, that the Smartform-2 should be loaded
