Automate action plan

Hello,

I am trying to automate action plan for opportunity object using salesforce flow. The criteria is that when an opportunity is created or updated with specific stage values then the action plan should be created. However upon testing the flow the action plan does not created and on debugging the flow I am getting the following error: This error occurred when the flow tried to create records: REQUIRED_FIELD_MISSING: Required fields are missing: [ActionPlanTemplateVersionId]. Although I have fetched the required field in the create record element in the flow. Please point out what am i missing.
