We ran an engagement studio where first we send an emails followed by check on if they have opened the emails with a wait period of 2 days. However I noticed that the Email opens on the Email send and the Email Open Action were not accurate. Please refere to screenshot below. I want to understand why this might be.

In addition to what Bill said, if you are comparing metrics available within email template report you may be seeing opens/clicks/sends to other sends (including test emails you sent) that you did with the same template.
I also had huge mismatch with clicks/opens due to the pre-defined IP spam filters.
