Can you confirm if you are trying to add the tab on a 'VisualForce Page + Tabs' template or a navigation menu item on a Community Builder based template?
If you are using the 'VisualForce Page + Tabs' template, you can add it by accessing the Community Administration page to set the tabs. For Community Builder based templates, you may refer to the documentation below:
