Hi I started to customise the lead form page layout but now want to revert to the default saleforce template. Is this possible?

Are you on Professional Edition or lower? That's going to be tricky because you only get one page layout. You'll probably need to start taking screenshots before you start changing things so that you have a point of reference if you ever need one
