Process Builder - Create new record based on several criteria

I am trying to create a process using Process Builder to create a new record and if numerous criteria are met from the "master" record then check box field will be checked on the new record. 

 

Currently the new record is not being created and I'm not sure I have the correct "Values" set for a few of the conditions - (#9 & #12 on the image)

    1. Make sure the values are set correctly. No double quotes needed around string values.

     

    2. Make sure the checkbox under Advanced on the Process criteria page is appropriately checked.
