I have an active workflow for Closed Won triggers, etc. but I can't figure out how to Field Update the Chatter Follow for everyone.

It looks like I gave you the link to the unmanaged app, whose tabs count against your limits. Managed apps don't have that problem and it looks like Cloud Swarm planned for that by making a managed version of their own:
