How do I run a report to produce all open opportunities that have a Miller Heiman Blue Sheet associated to them?

I want to get a quick report of all MH blue sheet opportunities

If you only need to see Opportunities where the field "Associated blue sheet" equals TRUE, then a custom report type is not needed here. Simply run an Opportunities report and add a field with this criteria. Your report will then return of the Opportuntity records meeting this criteria.
