Out of the box, Salesforce has very limited features available for Notes and Attachments functionality. In my implementation, we have strong use cases of private and public notes and hence we created custom objects for both to suit our security and notes requirements.
Normally, it is good to create custom objects to provide full flexibility to Notes requirements.
