 I invited a co-worker, he attempted to create an account, but he got a message saying his account had already been registered.  Is that because he already has a salesforce.com account registered to his email?

     Yes this is correct. If he already has a Salesforce account he will not be able to use Chatter.com but he will be able to use Chatter inside of Salesforce.
