@Mary Nabong - When I spin up the HTML in an org without My Domain enabled, the URL is:
https://webto.salesforce.com/servlet/servlet.WebToCase?encoding=UTF-8When I spin up the HTML in an org with My Domain enabled, the URL is:
https://mydomain--UAT.cs95.my.salesforce.com/servlet/servlet.WebToCase?encoding=UTF-8What I don't know for sure is if web to case will begin to fail once you enable My Domain. To be safe, I would swap the URL. Hope this helps!
