Line Graph in Account History Report

I've created an Account History report that captures the following fields:  Account Name, Account Owner, Field / Event (which is the custom field Account Score), Edited By, Edited Date, Old Value, and New Value.

I want to create a line graph that displays the initial Account Score, then subsequent scores by Date to view the trend.

I'd also like to embed that graphic in the account if possible.
