Person Accounts - Address fields

A user manually updated each Contact page address for every Opportunity who purchased between certain dates. He separated out the with the Mailing Street from the City, state, and ZIP.  Each part of the address was in its own field.

But, now all of these changes have reverted to  one field. The entire address has moved itself to the "Street Address" field. 

example:

Street address field:

123 Main St

Anytown, NY 12222

United States

How do I fix this?  Any feedback is appreciated!
