Hope someone can help.

I am trying to create a Data Source using SQL Server from Tableau Desktop (Latest version). The dataset size is 60G and I don't want to wait 4 hours to extract this locally and upload. So how can you create the definition, publish to Tableau Cloud and then use the server to extract?

I have read many support cases raised, one which looks promising is to create "Create empty Extract ?" boolean parameter on the local workbook and then Create and publish the datasource. Indeed this does create the datasource without data - However, a rather fundamental problem is that there is now way in the Cloud on a refresh to remove this filter. Any refresh you try after operated from the Cloud seems to have the filter baked in and there is no way to change it.

I have also tried to create a Live connection and then convert to an Extract, but you cannot do this on this type of connection (I did notice this was available for other types of connection that weren't SQL Server :-0).

Is there something I am missing? It would seem that Tableau is completely ineffective for large SQL Server datasets :-0.

You don't remove the filter, or even change the parameter value on the server. The parameter is used to make the extract empty, running the extract locally. Before publishing, switch the parameter to the opposite value, but don't re-generate the extract. You now have a blank workbook with the parameter set to "Update data". Publish the workbook. The parameter value is only checked when you generate the extract (because it's a data source filter), and on the server, the value now triggers a full extract.
