Hello @Swapanjeet Singh, for information the ProviderActivityGoal object is part of Agentforce Life Sciences' standard objects, available in API version 65.0 and later. However it seems that you have just created the instance, based on the sandbox creation date....
Otherwise, I guess you would need to open a case to support / circle back with your PAM on this topic if you are still blocked a few days after.
