Why does Contact Name sometimes appear as <surname> Household?

Now we have moved to Lightning we occasionally see that the full name of the Contact , e.g. John Smith, appearing at the top of the page reads instead 'Smith Household'. Nothing else appears to be wrong just this. Has anyone else seen this?
