I am trying to add multiple choice questions to my form but am not sure what HTML code to put into the confirmation so it feeds the multiple answers into the data extension and multiple answers appear on the data extension with the answers rather than just 1?

Ok, so the main issue is that you are missing a name attribute. So in each select, apart from id="xx" you should also add name="xx". For example:
