How do I create a custom Prospect Field in creating a new Forms

First you want to create a new prospect field as mentioned here: https://help.salesforce.com/articleView?id=sf.pardot_fields_create_custom_field.htm&type=5

Then, this field should then be available to include on a new/existing form.
