I am a Salesforce admin and We have recently implemented CPQ and I am getting requests from sales managers/ sales team etc asking to change fields, add new objects, hide fields because they dont like something or they have previously worked with Salesforce in another company who did things differently etc. To make sure these changes are authorised and in line with the process, I am looking to create a request form, where users can add their names, change they want with details. I then want this form/changes approved by the CFO, Global head of Sales, CEO. So we have an approval process for this?

Yes you can do in that approach and for sending Approval you have to create multiple approval.
