Activity History Notes Missing

We have several users report their Activity History NOTES are missing for entries created in October-December 2015 (entries are still there just the notes are missing).  Notes made after 1/1/16 and before 10/1/15 are not missing the notes.

 

Any idea what happenend and more importnetly how we get those notes back?

    Fortunately we have not had it occur again.  So for now we do regular data exports on the off chance data goes missing again.
