When you upload an outbound change set, are the versions of the items within from when you added them to the change set or at the time you do the upload?

Say if I added a flow definition (process) to an outbound change set but amended it into a new version before uploading the change set - do I have to remove and add it to the change set again?

Yes, For all the components that we have added to the changeset and after if we will make changes the latest changes will be uploaded from the change set.
