What is the excel template needed for a mass upload of accounts & contacts?

What is the excel template needed for mass upload of accounts and contacts? Is it possible to upload account and contact information directly from excel? How can we prevent duplicates? I would like to mark all of the new accounts and contacts under the same heading, "prequin real estate 2013." Thank you.
