    Hi Trent

    Use the below:

    IF(Linked_Dog__r.Annual_Vaccination_Due_C3_or_C5__c   <  today()+45 && Linked_Dog__r.Annual_Vaccination_Due_C3_or_C5__c  >=  today()  , "Annual Vaccination - C5"  ,

    IF(Linked_Dog__r.KC_Vaccination_Due__c  < today()+45 && Linked_Dog__r.KC_Vaccination_Due__c >= today()  , "Kennel Cough Vaccination"  ,

    IF(Linked_Dog__r.Next_Heartworming_Treatment__c  <today()+45 && Linked_Dog__r.Next_Heartworming_Treatment__c  >= today() , "Heart-worming Treatment"  ,

    IF(Linked_Dog__r.Next_All_Wormer_Due__c  < today()+45 && Linked_Dog__r.Next_All_Wormer_Due__c  >= today() , "All Wormer Treatment"  ,

    IF(Linked_Dog__r.Next_Flea_Treatment_Due__c <  today()+45 && Linked_Dog__r.Next_Flea_Treatment_Due__c >= today() , "Flea Treatment"  ,

    IF(Linked_Dog__r.Next_Tick_Treatment_Due__c  < today()+45 && Linked_Dog__r.Next_Tick_Treatment_Due__c  >= today() , "Tick Treatment",

    ""))

    ))))
