For this package to work (to allow external users to complete the surveys) I have to create a Force.com Site.
I have done this (see below details)...
However, when I click Preview next to the Active Site Home Page (TakeSurvey), I am presented with this...
It should render questions related to a survey but, this is not the case. This is my first time working with Sites, so help would be appreciated. I assume I have something wrong in terms of set up withe the Site.
The Survey link reads...
But. when Open Survey is clicked or I copy and paste into my browser, I get the following...

No need to make change in the VF Page if its working fine.
You can check Field Level Security for the fields you are expecting to see and just a guess try to uncheck all HTTPS option and check again
