Hi Bob,
There are two types of logos based on your screen, on upper left corner in a salesforce org. One logo appears when you're in an app screen in salesforce and another one appears when... basically overall org including setup pages.
1. To change the logo for your salesforce org, you can to go to :
Setup > User Interface > Themes and Branding
Here you can't modify the built-in themes, but you can create a new one and set your logo in the new theme and activate it. Here's a link of help doc for this :

2. To change the logo for an app, you can follow as @Srikanth mentioned in his steps. But, in this case, you have to remember to also select the checkbox that says 'Use the app's image and color instead of the org's custom theme' on 'App Details & Branding' page, to override org's theme and get your app logo and other app themes visible.
