We have been using Salesforce for several years with rather involved customizations developed by consultants. Our development team is now very interested in the NPSP and I want to make sure that there are no conflicts between our customized instance and the NPSP. Is there a step by step guide for reviewing new apps in a sandbox? As an aside, we do not currently track donations or grants in any program so no involved import is necessary - just spreadsheets.

This may not be the answer you want, but I suspect that you will be well-served by engaging a consultant. That's based on two details you mention:
1) "rather involved customizations"
2) converting an Org that has been used for "several years" may require some data conversion. Do you know your existing data model? Person Accounts? 1:1? Bucket? Other?
I haven't done this upgrade myself but if I were trying to do it on my own, then I might look toward using a full or partial-data sandbox so I could study the data implications outside of Production.
