Add Opt-In Checkbox to Pre-Chat Form

I can't seem to find a way to add an opt-in checkbox to our live chat's pre-chat form in Salesforce. We'd like to capture opt-ins from live chat and of course have this sync to Pardot prospect profiles. Has anyone done this before?
