Can't get Approval Process to work from Process Builder

I've got a process builder that has a step to submit for approvals. I can get it to work in Sandbox but not in production. I've made all the recommended changes I can find in the forums but nothing seems to work. It doesn't error it just submits and rejects automatically. HELP!

    @Pattie - I have the same issue as the users. I've got syst admin permissions

     

    @John - I've double/triple checked the approvals and and the criteria and it definitely meets the criteria. The sandbox and prod look the same. 

     

    As part of the PB I have it change the owner to a queue. Could that be the problem? Even though it's the same in the sandbox.
