Make Text Field and (3) Picklist fields required on Opportunity?

I have a section on the Opportunity record to capture details for "Product Trial" requests - baseline field is a "Checkbox" the idea is if the box is checked and the remaining related fields are NOT populated an error message is triggered to remind the user to populate these fields. The formula I came up with does not trigger and I don't know why. I would appreciate any suggestions, here's my validation formula.

 

AND(

 

 Trial__c = TRUE,

 

 Trial_No_of_Bags__c = NULL,

 

ISBLANK(TEXT( Product__c )),

 

ISBLANK(TEXT( Formula__c )),

 

ISBLANK(TEXT( Package_Size__c )))

 

Error Message: SEE PRODUCT TRIAL REQUEST Section - Please enter number "Trial: No of Bags" and select correct "Product", "Formula" and "Package Size".

    Try this :

    AND(
    Trial__c = TRUE,
    ISBLANK(Trial_No_of_Bags__c),
    ISBLANK(TEXT( Product__c )),
    ISBLANK(TEXT( Formula__c )),
    ISBLANK(TEXT( Package_Size__c )))
