Is it possible to send a mass email to all accounts that have an attachment (Notes & Attachments) and include their account-specific attachement in the email?
No it is not - or at least not with standard Salesforce mass email functionality. You can include an attachment as part of a mass email, but it would need to be the same attachment for each recipient.
