customize how records get approved

Please help.  I keep getting this same error when completing the approval process for "Approve New Account."  The challenge is detecting it because the process fails yet my flow error email says its not finding "Flow API Name: Approve_Approve_Account."

 

This is the challenge error:

First exception on row 0; first error: CANNOT_EXECUTE_FLOW_TRIGGER, We can't save this record because the “Approve New Account” process failed. Give your Salesforce admin these details.
