WHAT I HAVE TO DO

@Unnati Singh Note : In the property pane, change the Label of the section to Support Plan Information 

• Make sure you have  Select Account Layout, while upgrading the Dynamic from
  • After upgrading the Dynamic Form, please check steps 13 to 16 in the screenshot above. 
  •  Here, you need to create a Drag and Drop Field Section component and change its Label name to 'Support Plan Information. 
  • Once you have done  you can arrange the filed  values as per module instruction 
• check the below screenshots:After chnage the lable name you can get below like:
