This occurs in both Production and Sandbox. If I create a new Donations-Opportunity Name, add the refresh name button to the layout, go to the donation, and click the Refresh Name button, then briefly I will see the new naming convention flash on the screen, and then the Donation name reverts back to the original format. This occurs with any of preset naming choices or in "other" custom names I've created.

Yes, that means that NPSP is firing correctly and something is firing afterwards. Workflow Rules and/or Process Builder are the first places I'd look.
