Templates for people requesting access to our data

I need to obtain a template that our company can use as a baseline for whenever an employee requests to have access to our data (in Salesforce.com).

Are there any good templates out there that I can use?

Also, is there a similar template for whenever a user or group of users requests to add or change or delete objects / reports in the system?
