How do I build a report that calculates the monthly average in a custom group field?

I'm trying to calculate the monthly average of cases a rep is doing. This is the data below. I also want to calculate the total average of cases every 3 months.

    Using the numbers in  your Report screenshot, what result would you expect the Formula to be?

     

    ( 40 / 4 ) = 10?
