Make your own FormAssembly Dynamic Picklist

Thanks to some interesting conversation on #FormAssembly App's forums, I've learned that you can use javascript to create your own dynamic picklist. This is really cool! The code that @Robert Wynter shared is a terrific start. But I'd love to get some collaboration going to make this easier for non-coding admins (like me) to be able to use. So I've created a GitHub repo as a place to collaborate on the code snippet. Please help us!
