I have an issue with Mapping the 3 fields as I cannot see the MAP botton once I do the selection. Please see picture below. Is there a way to fix this?

Might be a monitor resolution issue.
Try zooming out:
- Ctrl + scroll mouse wheel, or
- Ctrl + - (minus button)
