I'm working on a DupeBlocker setup right now and for some reason I can only get the rules to fire on updates of records and nothing on inserts. Has anyone seen behavior like that before (this is in NPSP 3). The rule is very simple: First Name, Last Name, and Personal Email.

Nope. :-) The problem with Dupe Blocker is that its trigger conflicts with the NPSP trigger that fires when a Contact is created. Native Dupe Management runs well before any triggers in the order of execution.
