The Lead Id is stored in the Parent Id of the FeedItem object.
I am able to use Get Records to retrieve all Chatter Posts (FeedItems) where ParentId is the triggering Lead Id.
However, when I attempt to use Update Records to change the Parent Id of a Chatter Post (FeedItem) from the Lead Id to the Opportunity Id I find that the Parent Id field is not available in the Update Records 'Set Field Values for the Feed Item Records' Field list (see image).
How can I transfer posts to another record?

You can do this by pulling FeedItems as noted above. It can be dangerous though depending on the volume of Chatter on the record. Flow can only handle so much. And then you have the Comments and Attachments to contend with. So this is feasible for limited Chatter, but can quickly scale beyond what Flow can handle.
